Preparing Your Mind for a Healthier Future

Preparing for bariatric surgery involves more than physical preparation. Weight-loss surgery can bring significant changes in eating habits, lifestyle, relationships, body image, and emotional well-being.

Mindful Horizon Mental Health provides comprehensive pre-surgical psychiatric and psychological evaluations for adults preparing for gastric bypass and other metabolic and bariatric procedures.

Our goal is to provide a supportive, professional evaluation that helps determine psychological readiness for surgery while identifying strengths, concerns, and areas where additional support may help you achieve long-term success.

Referred by Your Bariatric Surgeon?

If your bariatric surgeon, hospital, or weight-loss program has requested a psychological evaluation before surgery, you may contact Mindful Horizon Mental Health directly to schedule your assessment.

With your authorization, completed evaluation documentation and recommendations can be forwarded directly to your bariatric treatment team.

Patients may contact our office directly.

What Is a Bariatric Psychological Evaluation?

A psychological or psychiatric evaluation is commonly included as part of the preparation process for bariatric surgery.

The evaluation is not intended to judge you or look for reasons to prevent you from having surgery.

Instead, it provides an opportunity to evaluate your emotional and behavioral readiness for the significant lifestyle changes associated with weight-loss surgery.

During the evaluation, we will discuss your mental health history, eating behaviors, coping strategies, support system, understanding of your procedure, expectations following surgery, and your ability to follow recommended postoperative care.

The goal is to help you and your surgical team identify factors that may support a safe and successful surgical journey.

What Will We Discuss During the Evaluation?

Your comprehensive bariatric evaluation may include discussion of:

  • Your reasons for pursuing bariatric surgery

  • Previous weight-loss efforts

  • Current and previous eating patterns

  • Emotional eating or stress-related eating

  • Binge-eating behaviors, when applicable

  • Your understanding of your planned bariatric procedure

  • Expectations regarding weight loss after surgery

  • Understanding of dietary and lifestyle changes

  • Depression, anxiety, trauma, or other mental health concerns

  • Current and previous psychiatric treatment

  • Psychiatric medications

  • Alcohol, cannabis, nicotine, or other substance use

  • Coping skills and stress management

  • Family, caregiver, and social support

  • Body-image concerns

  • Relationship or lifestyle changes that may occur following surgery

  • Ability to attend follow-up appointments

  • Ability to follow nutritional, medical, and postoperative recommendations

Standardized mental health screening tools may also be used when clinically appropriate.

“I Have Depression or Anxiety. Will That Prevent Me From Having Surgery?”

Not necessarily.

Having a history of depression, anxiety, trauma, or another mental health condition does not automatically mean that you cannot undergo bariatric surgery.

Many individuals pursuing weight-loss surgery have experienced mental health concerns at some point in their lives.

The evaluation helps determine whether current symptoms are adequately managed and whether additional treatment, education, or support may be beneficial before or after surgery.

Our goal is to understand your individual situation—not to expect perfection.

You Do Not Need “Perfect Answers”

Some patients feel nervous before their psychological evaluation because they worry about saying the wrong thing.

There are no perfect answers.

Honesty is extremely important.

The purpose of the evaluation is to understand your current strengths, challenges, expectations, and support needs so that you are better prepared for the changes that may follow surgery.

Your evaluation is an opportunity to discuss concerns openly in a confidential and supportive environment.

Questions You May Be Asked

During your appointment, we may discuss questions such as:

Why are you considering bariatric surgery?

What health or quality-of-life improvements are you hoping to achieve?

What do you understand about the procedure you are having?

What eating changes will you need to make after surgery?

How do you currently cope with stress?

Do you sometimes eat when you are anxious, depressed, lonely, bored, or upset?

Who will support you during your recovery?

Have you experienced depression, anxiety, trauma, eating concerns, or substance-use problems?

What are your expectations regarding your weight after surgery?

How do you plan to maintain the lifestyle changes required after surgery?

These questions help us understand how prepared you are for surgery and identify any additional resources that may be beneficial.

What Should I Bring to My Appointment?

Please have the following information available when possible:

  • Photo identification

  • Current medication list

  • Name of your bariatric surgeon

  • Name of your bariatric program or hospital

  • Surgeon or program contact information

  • Any forms provided by your bariatric program

  • Mental health treatment history

  • Insurance information

If your surgeon provided a specific psychological evaluation form or checklist, please bring or submit it before your appointment.

Mental Health After Bariatric Surgery

Bariatric surgery can result in major physical changes, but emotional and behavioral adjustments are also important.

After surgery, some individuals experience changes involving:

  • Their relationship with food

  • Emotional eating

  • Stress management

  • Body image

  • Self-confidence

  • Family and romantic relationships

  • Social activities

  • Expectations regarding weight loss

  • Depression or anxiety

  • Coping with plateaus or changes in weight

For patients who need additional support, behavioral-health treatment can be an important part of maintaining long-term wellness.

Mindful Horizon Mental Health may provide ongoing psychiatric medication management, mental health follow-up, and supportive behavioral-health services when clinically appropriate.

Frequently Asked Questions

Why do I need a psychological evaluation before bariatric surgery?

Bariatric surgery requires significant and permanent lifestyle changes. The evaluation helps your treatment team understand your psychological readiness, expectations, coping skills, eating behaviors, support system, and any mental health concerns that may need additional attention.

Is this a pass-or-fail test?

No. The goal is to complete a comprehensive individualized assessment and determine whether there are psychological or behavioral factors that should be addressed as part of your surgical preparation.

Some patients may be ready to proceed, while others may benefit from additional treatment, education, or stabilization before surgery.

Does having a mental health diagnosis automatically prevent surgery?

No. A diagnosis such as depression or anxiety does not automatically prevent someone from being considered for bariatric surgery.

The severity of symptoms, current stability, treatment adherence, safety concerns, substance use, and ability to participate in postoperative care are considered individually.

Will you tell my surgeon about everything we discuss?

Your privacy is important.

Information and recommendations related to your surgical evaluation may be provided to your bariatric treatment team when you authorize communication.

Applicable privacy and confidentiality requirements will be followed.

Do I have to be referred by a surgeon?

Patients may contact Mindful Horizon Mental Health directly to inquire about scheduling.

If you have already selected a bariatric surgeon, please provide the surgeon or program information so that required documentation can be coordinated appropriately.

Can my evaluation be sent directly to my surgeon?

Yes. With your written authorization, completed documentation can be forwarded to your bariatric surgeon or weight-loss program.

What if my surgeon has a specific form that needs to be completed?

Please provide the form before or at the time of your evaluation.

We will review the bariatric program's requested documentation requirements and complete applicable sections based on the findings of your evaluation.

How long does the evaluation take?

Appointment length may vary depending on your history, clinical needs, and the requirements of your bariatric program.

Please allow enough time for a comprehensive interview and any necessary screening tools.

Do you accept insurance?
